To clarify, are you having issue in importing your Fillable PDF form?
May we know the error message you have received when importing the PDF Form?
Unfortunately, we cannot view attachments in this thread.
Please make sure your are importing a fillable PDF.
Can you please try another web browser in importing your Fillable PDF?
