I need to change a user name and email address.

We can change your username before we do that, please provide at least three usernames you wish to have.

Also, please note the following if the username changes:

-Custom URLs names (i.e. : http://www.jotform.com/OLD_USERNAME/myform ) will not work once we change the account username. You have to manually update the form URL.

-However, direct form links like http://www.jotformeu.com/form/53015216247346 will be fine.

-Please provide alternate names in case the proposed username is not available.

If you want to change the email, you can do that from the account setting page.
