Having issues embedding the form in my web page.

Hi, 

My website is hosted on Cloudflare through TherapySites and I was informed to send them the embed code so that the application I created would be accessible on my site.  Unfortunately the form isn't working and looks like a picture.  How do I link the form with an "Apply Now" button?  Thanks!
