    3. Submission are being received in different formats for the same form. on some occasions there is only a PDF attachment and the photos are within the PDF, other times the photos and PDF come though in one email with multiple attachments. can you please explain why this happens and if there is a way to make the submissions come in one email with multiple photos and the PDF of the form attached?

I have checked your form and it seems you have an upload field.

In your notification email, you will receive the file upload as attachments as well as the submission PDF document.

Example: the three highlighted are upload files

However, your upload filed is not required. If your form user doesn't add an upload file you will only receive the PDF document.

You may also disable the file upload to be attached in the notification email.

Go to your notification email>Edit>Advanced and set send upload as attachment to NO.

    Hope this information helps.

just to confirm, if i change the 'send upload as attachment' to no, will i just receive the pdf form with the photos in the pdf or will i receive the pdf document and if there are photos within the pdf document they will be received via email as attachments with the pdf form?

That option is only for the uploaded files excluding the PDF Submission Document. So setting it to NO, the uploaded files in your file upload fields will not come as attached files on email. However, you will see them on the email content as well as on the PDF.
