submissions are not forwarded to SalesForce

Hello, so I have a custom contact form that syncs contacts who fill it out to Salesforce, sometimes contacts do not sync, there is no pattern that we can decipher and this is causing us issues. 

Can you please try re-embedding your form using the Iframe code?
