Account suspended for some reason.

    Hi Jotform,


    Can you please assist with my account? I am unable to login as it says my account has been suspended.

    Your account is suspended because your form 2019 NRC Player Info Survey is used to collect passport info without user's consent, which is against our Terms of Use


    Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    Collecting Sensitive Information. You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

    If you want to collect passport info you have to add mandatory Terms and Conditions and Signature widgets or enable form encryption on your form.

    Let us know if you agree with our terms and we will lift the suspension of your account.

    Hi Joanna,

    Thanks very much much for your quick response. I hadn’t read those terms close enough. Thank you for highlighting that for me.

    I do agree to the terms you have outlined in order to collect passport information. If you could please assist in lifting the suspension and then assist with my forms so that passport information is able to be collected that would be greatly appreciated.

    I reactivated your account. Please add the Terms and Conditions and Signature widgets or enable form encryption as soon as possible.

    Thanks for your help Joanna. I will get to that asap. I am currently traveling so unable to access it properly. But I will get this sorted very soon.
