I did update my Ipad software to the newest, still dont work, I also cleared my coockies and internet data. Is there anything else I can do? Please reply to my email.

I tried viewing your form using the mentioned devices here but they appears to be working fine. It also works properly on PC. I would suggest to review everything on your side. Thanks!
