Thanks for your reply. I tried that way, but it did't work: The form came out differently after I clicked Clone Form. Since the difference is too great, I couldn't use it as a template. Please see attached image.

The form you are trying to clone is a card form, you must select the correct or the same form layout of the form you are trying to clone.

You can also change the layout of your form in the form settings, please follow the guide below.
