We have had people tell us they submitted the form but we never receive it. It doesn't show up under the new submissions. It doesn't happen with all of them but it happens too frequently. It is very important that this form works 100% of the time otherwise we will be looking for another solution.

Your form settings were fine but I suggest you to check this article for more detailed information on why you haven't received any notifications from your form: How to setup email alerts to prevent "emails not being received" issues.
I have also cleared your forms cache and fixed your submissions count, please kindly check it again.
