Email Notification: Duplicate Field

    Why is this field being duplicated after it is sent? How can we correct this?

    Is the issue related to your Exit Kickoff form?

    It seems that the email template was modified. You can edit the question label of the second field to "Outgoing Associate email' inside of the Email Notification table, to resolve the issue.

    Or you can delete the existing Email Notification and create a new one.

    It is correct on the form in Jotform but is showing incorrect when it is emailed out. Is there an additional field that needs updated?

    The question label of the first and third field in the email table is not exactly the same as on your live form. The fifth label should also be updated from "Outgoing Associate Job Title" to "Outgoing Associate email".

    The easiest way to resolve the issue is to delete the existing Email Notification and create a new one. The new Email Notification will have the correct question labels.
