My Tiered Membership Application Notification Does Not Include All Fields

When receiving the email notification for our new application form (Tiered Membership Application), the notification does not include the application fields.  Our old application notification included all fields.  See attached samples.

Please delete the existing Email Notification and create a new one. The new Email Notification will have all the form fields.
