Approval Process Workflow

My answer got deleted when I clicked submit. I'm basically looking for a step-by-step of how to set up a form like this:

- The 1st person fills out the form and clicks "submit"

- That submission goes directly to their boss who can review and edit the form

- When the boss clicks "Submit" I want those final answers to come to me. I would also like the 1st person to get a notification that everything is complete

Is there a way to do this? I looked at the "A General Approach on Building an Approval Process Workflow", but it gives so many different re-directs that I couldn't figure it out.
