I want to disable the 2nd box on a multiple choice question

Hello this is for Original Medical History template and i am trying to set my parameters for the multiple choice section below called  "Health Plan Participation" If the person choses the box "I elect coverage" i want to prevent them from also checking " I decline coverage" and vice versa. How do i accomplish this?  Phone Number *

Job Occupation/Title*

Annual Salary *

Average Hours worked per week *

Health Plan Participation *

I Elect CoverageI Decline Coverage
