You can create a multi-page form with JotForm. You can use either our classic or card form layout options.

If you choose using a classic form layout, you can divide your form into pages with page breaks. In order to add a page break, click the "Add New Page Here" button in the form builder.

You can try this demo form to see the multiple pages in classic form layout: https://form.jotform.com/91963110309958

In addition, you can create multi-pages while using card form layout. The aim of the card form is to show one question per page but you can add more than one form elements to one page with the "Multi-line Question" element as follows:

You can also clone these forms and edit them. To learn about cloning a form, you can check out this guide: https://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL
