Is there a way to pass a unique code to the question field based on the results of 2 previous questions and populate the form automatically?
For example:
question 1: select location (location 1, location 2)
question 2: Select type (Type A, Type B)
In the spreadsheet to form, I assign the following unique codes:
Unique code1: 1A = (location 1, Type A)
Unique code1: 1B = (location 1, Type B)
Unique code1: 2A = (location 2, Type A)
Unique code1: 2B = (location 2, Type B)
Based on the answer to the first 2 questions I can pass the unique code to the spreadsheet to form question which will then populate other fields.

I was able to pass a value to the Spreadsheet to Form widget with a condition. So, if you have two options in each question, you should create 4 conditions (2 IFs each). Then you can pass the value, which you want to the Spreadsheet to Form widget. You conditions should look like something like this:
