How do I assign a value to a matrix?

    Hi,

    If you see my Sample Performance Form you will have an idea of what I am putting together. I am trying to create a performance feedback form and need to be able to tally the results in such a way as I achieve a total score (preferrably a percentage). This form would be used by a trainer, colleague or manager to observe and report on performance standards in order to encourage improvement and self awareness.

    Essentially I need help with:

    Assigning a value to each of the column headings for the matrix.

    Some of the matrix's I need are:

    Strongly Agree, Agree, Disagree and Strongly Disagree,

    Yes, No

    Always, Inconsistently, Never

    I also need help assigning a value to each matrix within the form i.e. one matrix may be worth 30% of the overall score while another is worth only 10%.

    Building a report that demonstrates these results

    Ideally I would like to have:

    One form completed and a report automatically produced

    The ability to select several forms created for that particular team member and create a report that tally's the results.

    Hi Rian,

    Assigning a value to each of the column headings for the matrix.

    Not quite sure what you mean with "Assigning a value", are you referring to column header labels? If so, it's on the matrix property tool bar.

    However, if you mean number value then have it calculated on the report then sorry but that's not possible.
