Embedded form on my teams website, submit button doesn't work

Good morning, 

I have created a form for our athletic team website, and the form works when just using the link, but when I embedded the form into a page on our website, (by copying the source code and inserting it via the "Insert Source Code" option on our website builder, the form comes up just fine and I can fill it out but when I hit the SUBMIT button, nothing happens.  It's like the submit button does not have any functionality.  

Although, when hitting the submit button, it does do validation because it will highlight the required fields that are left blank.  It just doesn't seem to submit the form and I never get an email from the embedded form.  I do, however, get an email if I just click on the link and fill out the form that way.  

But I do want to embed that form into our website.
