How can I have my account enabled?

    Hello can you help enable my account.  It has been disabled and I would like to get back in and delete the problem forms and create a new form.

    Hi there,

    I have just enabled the account, but I have deleted the fields in the form. You have file uploads that asked for scan drive license ID. For the part where other information is asked, including the SSN, we allow that, but you have to do the following changes:

    1. Add an agreement in the form for users to confirm/allow/agree that you are collecting the information for business purposes.

    2. Add an e-Signature widget for them to sign.

    We also recommend using a business email in the account for this type of account. That is part of our verification process.
