Using Quiz Jotform addon but it will not send email results

We are using the quiz addon for our procedural sedation jotform and in the past we had it set-up to email the results out once they were completed. This was working for the longest time but now it will no longer send out the email after the users submits the quiz.

I cannot replicate this problem with our Quiz app. Kindly check your spam/junk folder. The email from the app may have been filtered to your end, and that needs to be marked as not spam to whitelist us.
