Forms not working and can't log in

    Your forms are not working and you are not able to log in because your account is suspended. Your account is suspended because your Credit Card Authorization Form Template from is used to collect credit card info using an Upload field, which is against our Terms of Use.

    Phishing. You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    Card number, expiration date, security codes and card holder name are considered critical information and cannot be collected on plain text fields or with upload fields. The only services allowed to collect such data are those that are DSS/PCI certified.

    If you want to have a payment field inside of a form, you need to use one of our payment integration.

    Your Liability Waiver / Rental Agreement form is also violating our Terms of Use. In this form you are collecting Driver's License or Passport using an Upload filed. 

    Driver's License or Passport can only be collected with a pain text field if you add Terms & Conditions and Signature widget to the form or if a form is encrypted. 

    We will only lift the suspension on your account if you agree with our terms and allow us to delete the offending fields.
