release form won't submit

I linked my release form into Wordpress in my website. When I had someone try to use it, after they hit the submit button, I get a "please wait" message. I just tried one 5 minutes ago and I still see the message.  I looked in my reports, and I can see the form has been accessed many times but no submissions. Any idea what I did wrong?

Can you please try to use the IFrame method to embed the form? You can check and follow this guide:
