And I'd like to do the same for my business. I checked the element (code) and found that it was made with your app but I have no idea how to do that. Can you confirm if this is from Jot Form and if yes May I know if you can explain me what can I do to do this kind of help Center?

Yes, I can confirm that the form that starts with the question, "Is your question regarding an existing order?" and is embedded on that website you linked using the JS Embed script is created in JotForm.

The fields on that form are shown based on the answer provided in another field. That sequence can be created using condition logic that can be set up in the form.
