Why can’t I log in my account?

Couldn't logged in the account with the correct password. Account was suspended. 

I have checked your account and you have some fields of your forms which is not allowed according to our terms. So there are cases of rule violations in the form fields you created.

The account and form were already suspended due to violation to the Terms of Use.

To reactivate your account and have the forms compliant with the terms, I have deleted the fields in the form that were in violation.
