I am not sure if this has been asked, if so please direct me to the question. I am trying to have the answers of my form, moved to a customized pdf that is in letter format. Currently I am doing this with google forms but it doesn't allow uploading and attaching documents to their forms.
For example
Form Questions
Name - Bam Bam
address - 1234 Rubble St, Nowhere, FS, 111111
Email - bam@bam.com
PDF format
Hello, My name is (Bam Bam =Name). And I live at (1234 Rubble St, Nowhere, FS, 111111 = address). Thank you for interest in my name. You can email me at (bam@bam.com = Email).
Can jot form do something like this?

You may use the text element to get the form field values and attach the PDF document.
1) add your text element
2) Edit the text element and use the Form field button to get the values of the field.
