the telephone and mobile numbers dont seem to be coming through

I have made 2 test submissions to your form and I could see them in your submissions. If you still have problem could you let me know it with including your browser information.
On the other hand for Mobile Number area of your form you can use "Phone" area under "Quick Tools" or if you use "Text Box" you can set its validation as "Numeric" in "Properties" since I could submit some text as "Mobile Number".

I have cloned your form, filled out information and clicked on Submit. Everything seems to be fine with your Notifications. It appears that you have a number of Conditions on your form. Upon playing around with your form and inspecting the conditions, I have found one major glitch with the way you have set up your conditions.
If I select Quotation/General Information in the Information Request dropdown, the Mobile Number field appears
But when I go from Quotation/General Information to Booking In, the Request Ringback is still set to Yes, yet the Mobile Number field is now hidden.
Re-structure your conditions. Use only one condition for both Show Mobile Number field and for Show Email Adress field. Hopefully, that should make it work. Get back to us if it doesn't.
