Mobile Forms: WebKitErrorDomain

One of my assignee's receives a web kit error domain message when submitting a form. The iphone he is using has content filtering enable, but the website JotForm.com is added to the list of allowed sites so he can fill out and send in forms. All out other phones are setup the same way, but this one phone is the only one that has this issue. As far as I can tell it is not an issue with the phone. Can you help me figure out why this error is occurring?

Hello? Can anybody help? We allow jotform.com, but apparently that's not enough to be able to submit forms. Can any provide recommendations as to what other domains should be white-listed to be able to submit forms?

Please whitelist the domains and test once more if the issue persists please let us know. 

I added all the websites you listed to the content filter and it still wasn't working.

To better see what the url as that was being blocked, i sent myself a web link for the form, filled out the form, and notated the url when the error message appeared after pressing submit.

The address being blocked was "jotform.us". I added this to the allowed sites in the content filter and everything is working now.
