I logged in to Jotform.com and navigated to Account -> Billing -> Payment Method, but when I click "Change Credit Card" it simply opens a new, but blank tab.

I have switched your account type to Free. Please log into your account and then click on Premium option on the same page and re-upgrade your account.
Your subscription had email address instead of JotForm username. That's why that change credit card button does not work.
