How to design a booking form for guests to select the number attending for an event?

Hi I am trying to design a booking form for guests to select the number attending for an event then they fill in the pre order menu for that amount of people with names.

So when they enter say 10 guests, then 10 lines would appear on the preorder form, if 20 selected then 20 lines if 100 then 100 lines, etc.

This is my form so far and I cant seem to figure out how this could be done.

I believe that you are looking for the Configurable list widget. This widget allows to let the user enter information in each row as per your requirement. Please refer to the guide below for more information on setting up configurable list widget.
