It looks like comcast rejected messages coming from our servers , so your email addressed fell into our bounce list , as shown in this server log:email@example.com is IN the bounce list

I have checked your form, and I found that you did not map a field from within your form to the Sender Name
It only had "Please select" on it
That's why the sender name was blank and this was interpreted as spam message by Comcast, causing the rejection
