How can I assign a Unique Identifier for a patient to autofill?

    Hi! I am trying to figure out how I can assign a unique ID to each patient encounter from a procedure request form to autofill patient information on the Billing form. 


    Can you help me with this?

    You can use a Unique ID widget to your procedure request form. Here's a guide for that: How-to-Add-Custom-Unique-IDs-to-your-Form-Submissions.

    Then you can pre-populate the patient information from the above form to your Billing Form using a Spreadsheet to Form widget. 

    This widget allows you to upload your spreadsheet to your form and use the values you have on the spreadsheet to pre-populate your form.

    First, you need to download your procedure request form's submission as an Excel file. Then use that file in this widget (inside the Billing Form). The Unique ID will be your unique code to access the information from the said spreadsheet. The values are automatically populated on your form fields, as long as the spreadsheet column headings match your form field labels.
