My website is csflournoy.com. I have two jotforms on my site but only the survey is showing up incorrectly on Mac.
The survey looks fine and works great on PCs using Internet Explorer, Firefox, and Crome, But on Mac's using Safari the survey is all messed up. The questions are jumbled up (See Screen Shot) and the bottom is cut off.
I used wix.com to build the website - if that's helpful at all.Page URL:

Hi, I took a peek at your survey form on mac and I could see the Survey Form though I'm not quite sure what the issue you were having was since the screenshot you had did not show up. The only problem that I found was that the submit button was cut off. I have cleared your form cache along with the recommendation that you try embedding your form within an iFrame to see if this helps. If you continue to have problems please let us know.

The survey is now working on ipads but is still showing up incorrectly on imac.
I tried to attach a screen shot but it didn't work.
The problem is with question 3. Each word is starting a new line and the boxes below are push to one side. Also, the submit button is still cut off.

I have fixed it for your convenience. I just right click that field on the form builder and select "Expand". You might have "Shrink" the field.
