I would also like to create an addendum if needed on a form if needed. How would I do this?

Could you please advise what kind of addendum you would like to insert in your form?

If you have a screenshot you can post: https://www.jotform.com/help/438-How-to-Post-Screenshots-to-Our-Support-Forum

    Thank you!

For example. A form that has been submitted had an error or is missing
information. I would like to be able to add an addendum for the correction
so that the original submission is able to still be seen and unchanged (due
to fields being read only after submission)
    Does that help?

It's possible to edit a submitted form. We have a detailed help guide that explains how to edit a submitted form: https://www.jotform.com/help/325-How-to-Edit-Submissions-Made-on-Your-Forms

You may also include edit link on your email notification so it's even easier to access the edit submission page. By default, it's already present on the email notification at the bottom of your email.

Here is a screenshot from sample email notification :

You can refer to the following article if you still want to know how to add it to your email notification.

    I hope this helps
