When I fill out my form to test whether it forwards to my website as I planned, I get the message SORRY ONLY ONE ENTRY IS ALLOWED even when I put totally different names. Please advise.

You see the "Sorry only one entry is allowed" is because you have enabled "Unique Submission" setting in your form. Please check the screenshot below:
The "Unique Submission" as "Check cookies and IP" is preventing users to submit form again from same browser or IP address. I would suggest you to please select "No Check" in "Unique Submission" and that will solve your problem.
