Errors notice on form

When i add a questions to add flyer or document and make it a requirement its send an error message upon submission. 

We would like to apologize for any inconvenience. May we know which of your forms are you experiencing the issue you have described, please?

Also, with your permission, we would like to make test submissions directly on that form.
