Why my account suspended?

Our Reviewers Team looked into the form further and due to collecting credits on a free domain email. They have requested further proof of business as well as changing the email address to a corporate domain in order to unsuspend your account.

Do you mean the proof of the company e-mail address? or I'm not paperwork?

It means that you should use a corporate email domain(@mycompany.com) instead of a free email address(@gmail.com). 

Could you also provide the link to the webpage where you intend to embed the form?
