form error

    online forms were not forwarded to my inbox when patient completed them

    found form submission online but they are all blank

    please advise

    I do apologize for the inconvenience you are experiencing. Can you give us the form URL that you are not receiving the notifications for and also the form submission is showing blank online?

    So we can investigate this matter more for you.
