It doesn't give the slick "box" that previews and takes right off the form itself.
Any pointers?
If it is hosted on jotform, it works fine, but hosted on a private domain it seems to only want to allow to attach the photos??

Is it okay if we can see the website where you embedded the form?
Also, please try re-embedding the form using the iFrame method. Here's a guide: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code

Figured it out. I needed SSL. Your site needs to be https:// Once I installed the cert all was fine. For those "out there" who don't know what I'm talking about, you can typically buy SSL from your domain register. :)
