Adding a PDF to client's email confirmation

    I would like my client to have a pdf copy with their acknowledgement form so they can see what they have signed. 

    How do I do this?

    I would also like the last part of text that is the disclaimer appearing under the signature to be seen on the form details they receive with the acknowledgement.
