Have text in this box but when the submission arrives it only has the fields that someone filled in... appreciate hearing how to make this visible on my submissions report..the pdf if that is the only way to be viewed.

Also, when a jotform is put out for someone to complet does it show who the sender of the jotform is?  Like their email address or is that not displayed?

Please try enabling the Show Headers and Text options in the Submissions page. Here's a user guide: How-to-Show-Headers-and-Text-in-the-PDF.

In regards to your other question, I believe you are referring to the email notification. This is the email that you received when someone completed the form.

By default, the Sender Email of the notification is noreply@jotform.com. However, you can identify the person who filled up the form via the email content, email subject, and reply-to email.

    1. The email content shows the submission data.

    2. In the Email Subject, you can add the name field or email field parameter. Here's an example:

    3. If the Reply-to Email field targets the email field, then you'll see the submitter's email address in the Reply-to Email part of the email.

Hope that helps. If you have any questions, let us know.
