Our forms are no longer automatically populating in Box.net

Not sure what happened, but our application forms no longer automatically save to Box.net.  A folder is automatically created, but the the pdf form doesn't populate and then we have to manually place the adoption forms in Box.  This has not always been the case and we're not sure how to fix it.
