Cannot Login to account

cannot log into my account and have not been able to submit a question.  I believe this may be the reason - I may have forgotten to encrypt the form...

It looks like the reason you are unable to access your account is not due to the encryption of the forms but rather because one of your forms was going against our Terms Of Services. 

The form Small Business Credit Application Form was flagged. Can you please provide us with proof of your egit business Please send an email to reviwers@jotform.com  with the subject

I don't know what that form is that you are speaking of...I did not create a small business credit application form.  Please delete that and I will change my password.

I thought it may have been the Vendor on-boarding form...
