How to change responses to PDF Form

So I am in need of a form where I can get both a signature and the IP address. 

Unable to import form because the widgets aren't compatible. 

Created a blank form where I was able to add the widgets, but when trying to create those results into a PDF, its a 3 page packet. 

Our usual application is only a page in length, so I am trying to see if now that I have the results, can I transfer over to the original application?

Current method keeps changing the application because it is not compatible. 

Could you let us now what is the form in question? Is this the form? Please note that it is not possible to populate the form responses into an external PDF.
