Some information is not being captured in the google sheets integration

Radio button selection on first question, text box information for assistant coach, email text box information, and text box information for Base players, and the report headers do not match the forms headers.  Please advise

Were the fields label recently updated or those field modified after setting up the integration? 

I would recommend you to kindly try deleting the integration and adding it again to your form, this way the sheet updates, you can remove the integration by going to the settings tab, integrations section and selecting the Google Sheets option.
