Form not loading correctly

    Customer says:

     Yes, we’ve reloaded the page and used Internet Explorer as well as Chrome for browers – multiple individuals across different offices 

    I have tried two different browsers, and neither works.

    I can fill out all the form sections by hitting the “tab” key on my keyboard – but the “Registration Number” area seems to be broken for me.


    - please help
