Problema com formul�¡rio de cadastro

Existem três formulários na minha conta. O problema que estou tendo se encontra nos três. A plataforma está salvando o número de telefone do primeiro inscrito para todos os demais. Isso prejudicia muito o controle.

I tried to look up your account with the username Count_Jocum but could not find an account associated with that username. Could you please send us the username of the account or the Form ID OR URL to investigate this issue for you.

Just to clarify, the issue is that the subscriber's phone number is being saved to all submissions?
