Why is my JotForm Account suspended?

    I can see you are using the copyright logo, collecting sensitive information and asking for ID upload through your "Credit Card Authorization Form".

    Please note that it is not allowed to collect sensitive information such as credit card information, user login forms, etc using JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please check our terms of usage for more information on phishing forms.

    If you are associated with the business that you are using the logo for, you will need to send the documented evidence through to Jotform in order to use the logo on your form.

    In order to reactivate your account, kindly provide us the following information:

    1. The URL where your forms are used.

    2. Your website or proof of affiliation to a legitimate institution.

    3. Share with us a document that will prove that you're authorized to use the said logos.

    You can send the following information and email them to reviewers@jotform.com with subject line Thread #1918177

    We will let you know about the status of your account after a thorough review of your proofs.

    Or, we can just remove the logos, driver ID collection fields and upload fields to lift the suspension of your account.

    You can just remove the logos, ID upload and whatever to reactive the
    account because I need access to those forms right now to book my clients
    trip

    I have removed the offensive fields for your form and reactivated your account. Please refrain from collecting any such information other our phishing filter will mark your form as a phishing form and suspend your account. 

    You should be able to login to your account now. If there is anything else you need, please let us know.
