Submission doesn't work anymore > MySQL server has gone away

when making a submission the page keeps on loading until it gets to the following error:

I am sorry for the trouble caused to you. Are you still having issues with your form? 

It could be a temporary issue. I would suggest you to please test your form again and get back to us if the issue persists.
