Just in the last day, we've noticed that a few of the forms running on our Jotform Application now have bullets to the left of the questions.
This problem appears to have happened multiple times in the past, based on a search of the forums, but I could not find an example of it on an implementation of the applciation.
I dug a little bit and found the root of the problem: the correct css file isn't loading.
Here's an example of a form that is dispaying the error: https://webapps.uc.edu/JotForms/forms/ProvostForms/30713551451
Notice in the source code, that it's attempting to load css from https://webapps.uc.edu/JotForms/forms/static/formCss.css?undefined which doesn not exist.
Contrast this with a working form: https://webapps.uc.edu/JotForms/forms/ProvostForms/23030155515 which loads the css file from https://webapps.uc.edu/JotForms/forms/min/g=formCss?undefined

Hello, it seems the formCSS file in the static folder is either got deleted or does not have permission to be viewed or be accessed publicly. Please make sure to give permission to run or view the formCss.css. Thanks

There isn't a static folder in jotforms to begin with, nor is there an actual file called "formcss.css" this appears to a problem with the application.

Our knowledge is limited with regards to Jotform Application so I would suggest you to direct this issue to firstname.lastname@example.org.
