I think my Premium account was canceled because my credit card expired. How do I reinstate the "Starter" plan at this point? It's grayed out as an option. I have no forms currently being used.

Yes that is correct and actually I can already see that currently your account is on the Starter Plan again as of right now. You were probably originally on a Scheduled Downgrade which would explain why you saw that since if the payment fails it allows some time to update it but still schedules an automatic time on your account for when you will get downgraded.
