Hello,
I am writing because the university I work for in West Virginia is considering using Jotform.
We plan to apply for the nonprofit status.
I have learned that the purchasing office would require a formal quote before granting permission to apply for Jotform.
I have also learned that when we sign up for this service, the purchasing office would need to have Jotform sign a WV96 and purchasing afidavit.
Is this acceptable?

In order to get a quote, you can create a Purchase order in the portal. So, you will just create an order and use it as a quote. Then you can use different payment method later. Here is the guide:

Do you mean the W-9 form? We send the W-9 form to our users. We do not have the practice to sign or send any other documents. Usually, the W-9 form is all that you will need.
